Home
and community based services program designed to help elderly and disabled person to stay in their own homes
rather than going to a nursing home. Individuals 18 years of age and over and
elderly individuals who are a resident of any West
Virginia county, medically in need of nursing home care but wanting home care and meet financial guidelines are eligible
for services. Case Manager assigned to individuals to assess needs and broker all
resources and services needed to
fulfill needs. Medical equipment, indigent prescriptions, Veterans’ in-home services, Workers’ Compensation in-home service and
older worker training program also available.
