Home and community-based services program designed to help elderly and disabled persons to stay in their own homes rather than going to a nursing home. Individuals 18 years of age and over and elderly individuals who are residents of any West Virginia county, medically in need of nursing home care but wanting home care and can meet financial guidelines are eligible for services. Case Manager assigned to individuals to assess needs and broker all resources and services needed to fulfill needs. Medical equipment, indigent prescriptions, Veterans’ in-home services, Workers’ Compensation in-home service and older worker training program also available.

The goal of the FAST program is to develop a statewide parent and youth support network that will empower families to participate in planning, management, and evaluation of their child's treatment and service needs. Eligibility checklist:

at least one of the following: 1) child has a Primary Mental Health (DSM-IV Axis 1) emotional and/or behavioral diagnosis; 2) child's level of disability requires multi-agency intervention to improve conditions (services from at least 2 or more child-serving systems; one must be Mental Health); 3) child has received Mental Health/Behavioral Health services for at least a year or these services are expected to last for more than a year.
Legal Aid of WV is a statewide non-profit organization that provides family-driven advocacy services to children and adults with mental or behavioral health needs.

Serving Fayette, Monroe, Raleigh and Summers counties and offers specialized programs to meet needs of children and adults experiencing emotional, behavioral, developmental, or substance abuse problems. Work to identify and treat problems, in order to prevent a more serious problem from developing. Services include: 24 hrs/7 days a week access; short-term psychiatric care; individualized care; medication management, administration and evaluation; intensive supervision, group and individual counseling.

Provides education, training, and career services for people with disadvantages, such as welfare dependency, homelessness, and lack of education or work experience, as well as those with physical, mental and emotional disabilities. A private, non-profit behavioral health care organization that provides metal health, psychiatric and social work services that assist individuals and families with resolving issues that interfere with normal, everyday functioning. Eligible individuals will have Medicaid, CHIPS, a referral from DHHR Social Services or a private insurance that pays for the services provided. Requests for services can be made Monday thru Friday from 8:00 am – 5:00 pm.

Medicaid wavier program designed to deliver services to individuals in their home and community surroundings as an alternative to receiving services in an intermediate care facility for individuals with mental retardation and related conditions. DHHR will determine eligibility; must meet medical and financial guidelines. Service available include day habilitation, speech therapy, physical therapy, prevocational training, supported employment, transportation to clients, adult companion services, respite care and community based day habilitation.

SSI provides cash benefits to eligible individuals with disabilities. Ages youth thru 21 years of age. Children who get SSI usually also qualify for free medical care through the Medicaid program.
There are both financial and disability eligibility rules. The financial rules count the parents’ income from salary or wages. The rules also count the parents’ resources such as savings or property to decide if child qualifies. The amount of money you have affects eligibility and how much SSI your child gets each month. The disability rules require children to have “marked and severe” functional limitations from a physical or mental condition. This means that your child cannot do the same things that another child who is the same age, but who does not have disabilities, can do. SSI also provides benefits to eligible adults with disabilities who do not qualify for earned social security. Youth and adults automatically qualify for Medicaid if SSI benefits are received. When calling for application, must have Social Security number.

Early intervention for children up to the age of 3 years who are experiencing a developmental delay or have been diagnosed with a physical or mental condition which has a high probability of resulting in developmental delay. There are no financial eligibility requirements. Services include audiologists, counselors, social workers, child development specialists, nurses, occupational therapists, physical therapists, registered dieticians, speech language pathologists, and vision specialists. May also include transportation necessary to receive other WV Birth to Three Services.
